Patients & Caregivers
Choosing hospice is often a difficult decision. We are able to answer your questions and dispel myths that exist regarding hospice care.
Hospice is a plan of comfort care for the patient who is making the end-of-life transition. Hospice cares for the person, not the disease. Hospice does not hurry death or prevent death. Hospice care reaffirms life, focuses on dignity, and brings comfort and quality to the remaining months, weeks, and days.
We are here – 24/7/365.
If two or more of these possible indicators of hospice eligibility are present, hospice should be considered, and discussed with the patient, the patient’s family, and the patient’s attending physician. Anyone can make a hospice referral. We will perform an evaluation to help determine if the patient may meet the criteria for hospice eligibility.
